The St. Lawrence Valley Dog Club will be holding a microchip clinic for companion animals on July 21 at the Richard Lockwood Civic Center from 10am-4pm.

Please have dogs leashed and cats confined in an acceptable carrier when attending the clinic.

The following information is needed in order to complete the registration form :

owner's name and contact information
animal's information, including registration number if an AKC registered dog, as well as birth date
an alternate contact person's name and telephone number
the regular vet's name, address, and phone number

The cost for the microchip clinic is $25 per animal, which includes registration with the American Kennel Club's Companion Animal Recovery (CAR) database.
